ADH is a therapeutic program that supports adults with disabilities and/or seniors who have nursing as well as activities of daily living needs (e.g., personal care, dining).
MENTOR Adult Day Health provides vibrant day programs for elders with complex medical or rehabilitative needs. On-site nurses provide medication management and daily care for illnesses such as diabetes and COPD. Services are tailored to each person’s medical, social and other needs so they can make friends while staying active and independent.
